Privacy notice

RelayPoint is operated by Centergy Systems. This notice explains the information collected during the current pilot and how it is used.

Information collected

RelayPoint asks about role perspective, tenure, approximate organization size, supervisory responsibility, operating-condition ratings, and optional written examples. It does not ask for employee ID or the names of coworkers. If you request a Centergy review, it also collects the contact information you voluntarily provide.

Purpose and consent

Assessment information is used to generate your results and, only after the submission consent presented to you, to support Centergy review or aggregate organizational analysis. Centergy does not sell assessment information.

Access and reporting

Access to stored responses is restricted to authorized Centergy reviewers. Client-sponsored reporting is intended to focus on aggregate patterns. Small groups may be suppressed when reporting them could make an individual reasonably identifiable.

Retention and deletion

Pilot submissions are retained only as long as reasonably necessary for review, validation, and the related engagement, with a default target of no more than 12 months unless a different period is agreed. You may request deletion of an independently submitted response by providing its RelayPoint reference code.

Your responsibility

Do not submit names, health information, trade secrets, regulated data, or other details that identify another person. RelayPoint is not an emergency, legal, medical, whistleblower, or formal complaint channel.
